Requirements: - Master's degree in Social work or similar - License in clinical social work - 1-2 years hospital experience as a clinical social worker
Skills:
· Conducts interviews, assessments, and evaluations of patients and family members to identify social, emotional, and economic factors which may interfere with obtaining maximum benefits from medical care. __
· Assess patients’ mental health needs and provide mental health care/support.
· Assists with case management services to secure resources for clients, families and significant others to achieve treatment goals.
_· Participate in multi-disciplinary team conferences to discuss and evaluate patient needs. _
· Writes medical/social case summaries and reports documenting the social/emotional assessments and diagnosis made and the treatment rendered in accordance with utilization review standards issued by healthcare regulatory agencies.
· Attends in-service training programs, continuing professional education, seminars and workshops to maintain active licensure status as a Licensed Clinical Social Worker in accordance with California state licensure requirements and to continue professional development in social work practice, mental health theory, in the effective delivery of healthcare services and specifically for care of patients with Sickle Cell Disease.
· Ability to work as a member of a team

· Ability to interact in an appropriate and professional manner

Responsibilities:
· Oversee the mental health needs of patients in the adult Sickle Cell Clinic
· Assist with coordination of care and need for resources
· Willing to enhance fund of knowledge as it relates to Sickle Cell Disease
· Attend and productively participate in staff meetings and trainings as needed
· Maintain high degree of responsibility for client confidentiality
· Maintain a pleasant, courteous, tactful attitude with all contacts
· Work with minimal supervision in exercising initiative in organizing and completing daily tasks
· Solve minor problems by using judgment based on knowledge of Institute policies and department practices and procedures
· Meet established productivity and task management standards for assigned duties
· Overtime is a condition of employment and may be required based on the needs of the Institute
· Responsible for other tasks as may be required for efficient operation of the department
Salary: $40.87-$45.67/hr.
